Reinstatement When a leave of absence is no longer necessary If recovering from an illness, please have the doctor fill out a designated certificate.
Withdrawal When the occasion occurs A doctor’s certificate is necessary.
Temporary Absence When the occasion occurs For a period longer than 3 weeks
Temporary Exit from Japan When it is necessary to leave Japan temporarily Do not forget to file for a re-entry permit before leaving Japan.
Change of surname or change of address When name or address changed Necessary when there is something urgent to notify you, so do not forget to notify us.
Reporting traffic accidents or burglary When it happened If you are involved in a traffic accident on/off Campus. If you are assaulted, or injured. Or if your property is stolen.
